Annals of Internal MedicineRecommended Adult Immunization Schedule, United States, 2024

Comprehensive Updates Reflecting Evolving Healthcare Needs and Vaccine Innovations

The Advisory Committee on Immunization Practices (ACIP) has released its updated 2024 Adult Immunization Schedule, marking a significant stride in public health efforts. This comprehensive guide, endorsed by major health organizations, reflects the latest recommendations and policy shifts in adult immunization. It’s a crucial tool for healthcare professionals, aimed at enhancing vaccine coverage and addressing the dynamic landscape of vaccine-preventable diseases and emerging health concerns.

Key Points:

  1. COVID-19 Vaccine Update: All adults are now recommended to receive at least one dose of the updated 2023–2024 Formula COVID-19 vaccine, with specific guidelines for immunocompromised individuals.
  2. Hepatitis B (HepB) Vaccination: Revised guidelines suggest HepB vaccine series for adults aged 60 and older, regardless of known risk factors, and emphasize shared clinical decision-making for those with diabetes.
  3. Human Papillomavirus (HPV) Vaccination: Clarifications made on HPV vaccine series completion using recommended dosing intervals, with additional resources for shared decision-making.
  4. Influenza Vaccine Composition: The 2023–2024 influenza vaccine includes updated strains, with quadrivalent vaccines being standard. Notably, egg allergy considerations have been simplified.
  5. Meningococcal Vaccination: Updates include the removal of Menactra from the schedule and introduction of the pentavalent meningococcal vaccine (Penbraya).
  6. Mpox Vaccination: Newly added to the schedule, with recommendations for Jynneos vaccination based on specific risk factors.
  7. Pneumococcal Vaccination: Minor edits for clarity on guidance and dosing intervals.
  8. Polio Vaccination: Revised guidelines for unvaccinated or incompletely vaccinated adults and booster recommendations for those at increased risk.
  9. Respiratory Syncytial Virus (RSV) Vaccination: Introduction of guidelines for maternal vaccination and shared decision-making for older adults.
  10. Tetanus, Diphtheria, Pertussis (Tdap) Vaccination: Clarification that a Tdap dose at age 10 can be counted as the adolescent dose.
  11. Varicella and Zoster Vaccinations: No changes in the routine recommendations.
  12. Revised Content and Format: Enhancements to the schedule’s cover page, tables, and appendices for better usability and accessibility.
  13. New Addendum Section: Summarizes updates to ACIP recommendations post-approval of the schedule.

A CDC study found that adult vaccination coverage remains significantly below national targets. For instance, only about 25% of adults aged 19-49 years received the recommended Tdap vaccination.
