Researchers in China and Singapore identified CMNV in iris tissue of 70 patients with POH-VAU, a persistent uveitis pattern featuring IOP exceeding 45 mm Hg in more than half of cases. The virus, previously documented in more than 20 aquatic species, had not been previously linked to human ocular disease.


Clinical Considerations

  • Most POH-VAU patients had multiple anterior uveitis episodes; recurrence occurred in 62.5% over 2-year follow-up
  • IOP exceeded 45 mm Hg in 51.4% of patients; 30% required antiglaucoma surgery despite treatment
  • Seven in 10 patients responded to ophthalmic steroids, IOP-reducing medications, and ganciclovir; 35.7% required long-term medication
  • CMNV load correlated with IOP elevation and spread more rapidly in adults 70 and older

Practice Applications

  • Consider CMNV exposure history in patients presenting with recurrent, hypertensive anterior uveitis of unclear etiology
  • Recognize that puncture wounds from aquatic animal appendages and daily raw aquatic consumption represent the highest-risk exposures
  • Interpret findings cautiously: this is a small case-control study with acknowledged recall bias; no validated diagnostic protocol yet exists for CMNV in clinical practice
  • Monitor for antiglaucoma surgical need in patients with refractory IOP elevation and uveitis pattern consistent with POH-VAU
