  • Unplanned extubation rates decreased from 1.307 to 0.344 per 100 ventilator days between 2018 and 2023.
  • Nurses stratified patients into low-, moderate-, and high-risk categories with corresponding prevention bundles.
  • Interventions included family education, ventilator-weaning support, positioning checks, staff awareness, and postevent learning reviews.
  • Only 24% of patients experiencing unplanned extubation ultimately required reintubation.
