Orforglipron (Foundayo), a once-daily oral GLP-1 receptor agonist approved for chronic weight management, was evaluated in post-hoc subgroup analyses of two phase 3 RCTs across premenopausal, perimenopausal, and postmenopausal women. These analyses were not pre-specified primary endpoints and should be interpreted with appropriate caution, particularly in smaller cohorts.
Clinical Considerations
- At the maximum 17.2mg dose, orforglipron reduced body weight by 12.8% to 14.4% across menopausal stages in nondiabetic women at 72 weeks; reductions of 8.9% to 13.6% were observed in women with type 2 diabetes
- More than 80% of nondiabetic women across all menopausal stages achieved at least 5% body weight loss; roughly 60% achieved at least 10%
- The perimenopausal cohort in ATTAIN-2 was n=33, limiting the reliability of that subgroup’s estimates in women with type 2 diabetes
- Waist circumference reductions were observed across both trials and all menopausal cohorts, though specific values were not reported in this analysis
Practice Applications
- Consider orforglipron as an oral GLP-1RA option for women with obesity or overweight across menopausal stages when injectable agents are declined or not preferred
- Recognize that post-hoc subgroup findings support counseling on expected weight loss ranges but do not substitute for individualized shared decision-making
- Integrate menopausal status into weight management conversations, noting that efficacy signals were consistent across stages in this analysis
- Monitor for standard GLP-1RA adverse effects; orforglipron’s oral formulation may affect tolerability and adherence profiles differently than injectables
