Reports increased markedly after 2020, with a peak in 2023. Voluntary reporting and incomplete clinical details limit incidence estimates, but the consequences documented include permanent structural changes.
Clinical Considerations
- Textural changes were the most common adverse event at 25%, followed by pigmentary alteration (18.3%) and subcutaneous fat loss (11.6%), the latter occurring almost exclusively on the face and submental area
- Rare but serious events included neuromuscular symptoms such as transient facial weakness, muscle spasm, and dizziness (3.1%) and vascular injury (0.9%)
- Reports originated from both clinical professionals (31.6%) and consumers or patient advocates (39.5%), suggesting meaningful underreporting from clinical settings
- MAUDE data reflect voluntary submission; true adverse event frequency is likely higher than captured
Practice Applications
- Counsel patients explicitly on risk of permanent textural, pigmentary, and fat loss changes before any RFMN procedure
- Recognize that face and submental areas carry the highest fat loss risk; adjust energy settings and technique accordingly
- Monitor patients for delayed inflammatory reactions and prolonged pain beyond the expected recovery window
- Adhere to FDA-cleared indications; off-label anatomic applications carry uncharacterized risk profiles
