As cataract surgery increasingly targets precise refractive outcomes, tolerance for biometric error has narrowed, particularly for toric and premium IOL candidates. Tear film instability and meibomian gland dysfunction are among the most frequent drivers of keratometry variability, but epithelial basement membrane dystrophy, early keratoconus, and measurement technique contribute as well.
Clinical Considerations:
- Tear film instability is the most common cause of inconsistent keratometry; ocular surface optimization should precede repeat measurements, not follow them
- AS-OCT epithelial mapping can reveal surface irregularities that routine topography misses, influencing both corneal power calculations and IOL selection
- Higher-order aberrations from corneal versus lenticular sources can be differentiated via wavefront analysis and ray-tracing, clarifying the optical origin of unexplained variability
- Averaging discordant measurements is less reliable than identifying and resolving the underlying cause before proceeding
Practice Applications:
- Evaluate the ocular surface first whenever biometric measurements are inconsistent before attributing variability to equipment or technique
- Consider preservative-free tears, lid hygiene, topical cyclosporine or lifitegrast, and punctal occlusion to stabilize the tear film before repeating biometry
- Integrate corneal topography or tomography when subtle irregular astigmatism or early ectatic disease is suspected
- Recognize that IOL power calculations may shift meaningfully after ocular surface optimization; repeat measurements before finalizing lens selection
