Recurrent ovarian cancer management continues to evolve with expanding therapeutic options and changing maintenance strategies. ASCO convened a multidisciplinary panel that reviewed evidence from randomized trials and systematic reviews to develop updated recommendations for platinum-sensitive and platinum-resistant recurrent disease.

Clinical Considerations

• Platinum-sensitive recurrence should generally receive platinum-based doublet therapy, including carboplatin combinations with paclitaxel or pegylated liposomal doxorubicin.
• Bevacizumab may be added to platinum-based therapy and continued as maintenance to prolong progression-free survival.
• Patients with folate receptor alpha-positive platinum-resistant disease should receive mirvetuximab soravtansine.
• Evidence remained insufficient to support or oppose secondary cytoreductive surgery across several recurrent-disease settings.

Practice Applications

• Consider platinum-based doublets as preferred therapy for eligible platinum-sensitive recurrence.
• Recognize bevacizumab maintenance as an option after response to bevacizumab-containing treatment.
• Integrate folate receptor alpha testing into evaluation of platinum-resistant disease.
• Interpret surgical recommendations cautiously given mixed randomized trial results.
