In a cluster-randomized trial conducted across multiple countries, a multicomponent clinical intervention was evaluated for the management of postpartum hemorrhage in patients undergoing vaginal delivery. The intervention involved the use of a calibrated blood-collection drape for early detection of postpartum hemorrhage and a treatment bundle consisting of various interventions. The study found that the intervention group had a lower risk of severe postpartum hemorrhage, laparotomy for bleeding, or death from bleeding compared to the usual-care group. The intervention group also had higher rates of postpartum hemorrhage detection and adherence to the treatment bundle.

The study followed nearly 143,000 women who underwent hysterectomy for a benign condition over a 20-year-period and compared outcomes between those who had bilateral salpingo-oophorectomy (BSO) and those who didn’t. Younger (under age 45) women who had BSO had a higher 10-year cumulative risk of hospitalization for CVD. Women aged 45 to 54 with BSO had a higher 10-year risk for developing cancer. Based on the findings, the authors suggest a cautious approach to ovarian removal in postmenopausal women and recommend preserving ovaries in premenopausal women without a high risk for ovarian cancer.

The CDC now calls for screening of all adults aged 18 years or older at least once during their lifetimes. And vaccination should be offered to all who have not previously been vaccinated, regardless of serologic testing results. The CDC is now also recommending screening during every pregnancy, preferably in the first trimester, regardless of a patient’s vaccination status or history of testing.

The mass in the young child’s head was a “malformed monochorionic diamniotic twin,” which means that the fetuses shared the same placenta but had separate amniotic sacs in the womb. The phenomenon of one fetus becoming enveloped by another is known as “fetus in fetu” (“parasitic twin”), and it occurs in approximately 1 in 500,000 live births. The malformed fetus usually appears as a mass in the abdomen of the other fetus, wedged behind the abdominal wall tissues. In this case, however, the mass appeared in the “host” fetus’s head and most likely arose very early in development.
