Dry eye disease (DED), ocular surface disease (OSD), and other cornea and anterior segment diseases can cause patients to experience a diverse range of symptoms. The cornea is the most densely innervated tissue and is therefore an important target for therapeutics and research. Transient receptor potential (TRP) channels are signaling channels that play roles in signaling physiological processes, and TRP melastatin member 8 (TRPM8) is a cold-sensing receptor found on nerve endings of the ophthalmic branch of the trigeminal nerve. TRPM8 channels sense cooling changes in corneal temperature and are activated by evaporative cooling and hyperosmolarity, leading to stimulation of tear production, which may be why eyes water on cold days and in patients with dry eye. Understanding the interaction between symptoms, ocular healing, and dysfunctions that occur in this process can help optometrists care for their patients more effectively.

When the eyelid shifts to an abnormal anatomic position, this is referred to as eyelid malposition. Sixty percent of symptomatic dry eye patients have compromised lid closure due to eyelid malposition. Regardless of the type of eyelid malposition, hallmark symptoms such as foreign body sensation, excessive tearing, redness, and ocular irritation can all occur. There are distinctions between signs and symptoms. Although symptoms can help identify eyelid malposition, a careful objective examination of the eyelids is essential for determining the malposition and recommending the best treatment.

The effectiveness of one’s dry eye disease (DED) clinic depends on staff involvement in both diagnostic testing and patient education for three reasons: (1) It enables ODs to see more DED patients and quickly prescribe the treatment(s) needed to provide symptomatic relief; (2) It emphasizes the significance of diagnosing and adhering to the prescribed DED treatment; (3) And it fosters staff loyalty to a practice because they are actively involved in patient care rather than merely checking off the boxes.

According to the most recent Physician’s Desk Reference, 22 of the top 100 best-selling systemic drugs in the United States have been linked to dry eye. Furthermore, eight of the nine systemic drugs secreted into the tear film have been linked to induce dry eye, including common OTC medications aspirin and ibuprofen. This article will review and provide common examples of many of the medications on this list by class, as well as discuss the mechanism of causation and offer management and treatment approaches.
