ACOG’s focused update to Committee Opinion No. 734 reverses its prior reliance on ultrasound alone for many patients with postmenopausal bleeding. The revised guidance recommends combining transvaginal ultrasound and endometrial tissue sampling during the initial evaluation of most patients to improve cancer detection.

• Previous ultrasound-first strategies demonstrated inadequate sensitivity for endometrial cancer and premalignant pathology in some populations.
• Rising uterine cancer incidence and mortality were major drivers of the guideline revision.
• Investigators highlighted particularly poor ultrasound performance among patients at risk for high-grade cancers and among Black women.
• Ultrasound alone remains acceptable only when all specified low-risk criteria are met, including a single bleeding episode and endometrial thickness ≤4 mm.
