ConexiantAre GLP-1s Linked to Optic Neuropathy?

ℹ️ Observational Association Only Evidence

A nationwide Swedish cohort compared GLP-1 receptor agonists with SGLT2 inhibitors in patients with type 2 diabetes. The analysis found a higher relative risk of anterior ischemic optic neuropathy, but few events overall.

Clinical Considerations

  • Event counts were small, with 62 cases among GLP-1 users and 64 among SGLT2 users.
  • Five-year absolute risks remained low, at 0.12% for GLP-1 users and 0.07% for comparators.
  • The association attenuated in metformin-restricted analyses, suggesting possible confounding by diabetes severity.
  • Subgroup analyses by A1c, smoking, and individual GLP-1 agent had wide confidence intervals.

Practice Applications

  • Interpret this as association, not established causality.
  • Recognize low absolute risk despite elevated relative estimates.
  • Monitor future neuro-ophthalmic safety data for GLP-1 agents.
  • Avoid overgeneralizing findings to obesity patients without diabetes.
