PSMA PET imaging plays an increasingly important role in evaluating biochemical recurrence after prostatectomy or radiation therapy. This prospective multicenter registry study examined whether repeating PSMA PET after an initial negative scan identifies clinically meaningful disease and influences treatment planning.
Clinical Considerations
- 210 recurrent prostate cancer patients with an initial negative PSMA PET underwent repeat imaging.
- The second scan detected disease in 56% of patients previously considered imaging-negative.
- Repeat imaging prompted a management change in nearly 50% of patients.
- Positive repeat scans were more likely among patients with higher PSA levels and PSA doubling times under 12 months.

- Recognize that an initial negative PSMA PET does not exclude recurrent disease.
