ℹ️ Observational Association Only Evidence
MOGAD is a rare inflammatory demyelinating disease that can involve the optic nerves, spinal cord, and brain. A meta-analysis evaluated plasma exchange outcomes across 10 studies and 1,368 attacks.
Clinical Considerations
- Optic neuritis accounted for 41% of attacks included in the pooled analysis.
- “Excellent” recovery after PLEX occurred in 81% of optic neuritis cases.
- PLEX-related adverse events were reported in 3% of patients.
- Observational designs limit causal interpretation and separate effects from steroids or spontaneous recovery.
Practice Applications
- Recognize MOGAD as a rare cause of acute optic neuritis and neurologic deficits.
- Interpret recovery rates cautiously because randomized evidence remains unavailable.
- Monitor steroid response early when vision or motor function is threatened.
- Consider neuroimmunology input when severe MOGAD is confirmed or strongly suspected.
