Medical XpressProstate Cancer Prognosis Linked to Prostate-Specific Antigen Levels Below Threshold

ℹ️ Observational Association Only Evidence

Multiple phase 3 trials have suggested that a PSA level below 0.2 ng/mL may represent the most prognostically meaningful treatment response target in metastatic hormone-sensitive prostate cancer. This study evaluated whether those findings persist in routine clinical practice using real-world Veterans Health Administration data.

Clinical Considerations

  • Patients achieving PSA below 0.2 ng/mL within nine months were 54% less likely to die during follow-up.
  • A ≥90% PSA decline alone was not associated with improved survival if PSA remained above 0.2 ng/mL.
  • The cohort included 4,890 veterans treated between 2018 and 2023 for metastatic hormone-sensitive prostate cancer.
  • Combination treatment with androgen deprivation therapy plus additional androgen-signaling blockade increased likelihood of achieving the PSA target.

Practice Applications

  • Interpret PSA nadir alongside percentage decline when assessing treatment response.
  • Recognize that deep PSA suppression may provide additional prognostic information.
  • Monitor patients failing to reach PSA below 0.2 ng/mL for potential treatment intensification discussions.
  • Consider these findings as supportive prognostic evidence rather than definitive treatment guidance.
