Bispecific antibodies are molecules that are designed to engage or bind to two distinct targets, bringing two distinct cells together. The majority of those that have received regulatory approval are T-cell-engaging bispecific antibodies. Blinatumomab (Blincyto) for ALL, teclistamab (Tecvayli) for multiple myeloma, and mosunetuzumab-axgb (Lunsumio) for follicular lymphoma are some examples of FDA-approved treatments. In terms of regulation, the PI3K inhibitor medication class had a difficult 2022. Participants in a historic FDA advisory committee meeting in April were warned about dubious safety and efficacy. Less than three months later, the FDA issued a duvelisib (Copiktra) safety warning about mortality rates. As a result, the FDA’s Oncologic Drugs Advisory Committee (ODAC) decided against recommending duvelisib’s approval for the treatment of chronic lymphocytic leukemia/small cell lymphoma (CLL/SLL). This study provides evidence that older patients with Binet stage A CLL, including those in the oldest age groups, can receive the Chronic Lymphocytic Leukemia International Prognostic Index (CLL-IPI). Because of the high median age in our study, we were also able to identify a relationship between del(17p) and age, which raises the possibility that age-adjusted prognostic tools could enhance CLL prediction.
