FNAIT occurs when maternal antibodies target fetal platelet antigens, leading to thrombocytopenia. Multiple mechanisms have been proposed, but the dominant pathway has remained unclear.
Clinical Considerations
- Macrophage phagocytosis identified as the dominant mechanism
- IgG removal abolished platelet destruction, confirming antibody-driven pathway
- FcγRIII blockade produced strongest inhibition of platelet clearance
- Complement activation observed in only ~20% of cases
- Platelet activation and desialylation were uncommon contributors
- Humanized mouse models confirmed preservation of platelets with FcγRIII blockade
Practice Applications
- Reinforces antibody-mediated macrophage clearance as central disease mechanism
- Identifies FcγRIII as a high-priority therapeutic target
- Supports development of receptor-blocking strategies and antibody engineering approaches
