Behavioral and psychological symptoms remain a major management challenge in moderate to severe Alzheimer disease. A systematic review and meta-analysis of seven studies found cannabinoid-based therapies were associated with reductions in agitation and overall neuropsychiatric symptom burden compared with placebo.
Clinical Considerations
- Neuropsychiatric Inventory total scores were lower with cannabinoid therapy versus placebo across four studies.
- Agitation measures improved, including lower CMAI-SF scores and NPI agitation/aggression domain scores.
- MMSE findings did not demonstrate consistent cognitive benefit, suggesting symptom control rather than cognitive improvement.
- Somnolence was the principal adverse event, occurring more than twice as often in cannabinoid-treated patients.
Practice Applications
- Recognize agitation reduction as the primary signal emerging from current cannabinoid evidence.
- Interpret findings within the context of small studies and heterogeneous cannabinoid formulations.
- Monitor sedation risk when evaluating cannabinoid-based treatment approaches.
- Consider current evidence as hypothesis-generating pending larger randomized trials.
